At Kaiser Permanente we believe in helping members with advanced illness attain the highest quality of life possible by providing them with care that respects their values and wishes. When an advanced illness is diagnosed or a condition progresses, we make it a point to understand what’s most important to them at the moment, and we’ll work with them by providing information and support.
The Care Management Institute supports Kaiser Permanente’s care for advanced illness by facilitating discussions among clinicians, providing evaluation and measurement tools, and spreading program development. Our work helps Kaiser Permanente staff:
- Address the member’s psychological, social, and spiritual needs.
- Help with family conflict and give emotional support by helping the family talk about difficult issues.
- Assist with planning and decision-making.
- Identify likely future needs including legal, financial, and health care planning, as well as community resources and services.
